1. What problem(s) would you like to address through potential grants? Please list problem(s) in the order of priority, need or concern. Describe the following a) The problem is b) The problem is caused by c) The problem long term without intervention.

2. What project activities would you like to develop to address the above problem?

3. Goal (s)/anticipated outcome(s): What would an improved situation look like? (Format document for additional goals, if applicable.)

5. If already available, how will best practices for the implementation of this project be identified and/or implemented?

6. Does this project involve training for school and/ or district employees? If yes, list additional resources needed to implement this project (For example, an assessment program, furniture, etc.).

7. If applicable, what provisions will be needed to provide training for all staff involved for year one and each remaining year of this project? Indicate provisions in estimated training hours and cost to provide the training.

8. How will you determine student and professional staff growth from this project?

9. Will this project require network resources (servers, computers, etc.) and/or Internet access?

10. Will this project have any impact on students after school hours? If so, will the project require resources to be used at home via the Internet?

11. Will this project require additional telecommunications wiring: Data i.e. (Computer), Voice i.e. (Telephone), T. V. i.e. (Instructional Television), Wireless LAN i.e. (802.11a, b, g,) or Security i.e. (Security Camera’s)? Please list the type of service that will be needed and the projected areas that may be affected.

13. Does this grant project involve additional support from other school (s), department (s) and/or organization (s); if yes, have you contacted them?

14. What is a role that you could play in the development of the grant proposal (e.g. proofreader, writer, word processing, graphics, budget development, etc.)?

15a. Will the implementation of this grant project involve additional personnel beyond your current staffing plan allocation (answer “yes” even it’s a partial unit or an extension of current personnel beyond the allocation)? If your answer to “a” is “no”, skip sections “b” through “d”.

15b. How will additional personnel be used?

15c. Will you be using an existing job description or creating a new one? If yes, contact Judy Butler in Personnel. She will check for approval to use an existing description or submit your new job description to the Human Resource consultant for evaluation and salary grade placement.

15d. Have you accounted for personnel in the budget section of this document, and has Judy Butler in the personnel department confirmed the accuracy of this budget entry?

Please remember that salary and benefits must be included personnel.

Please acknowledge that the district has no obligation to maintain additional personnel when grant funding expires and that you will inform candidates of the potentially temporary nature of the position.
