The Polk County School Board and Walgreens Health Initiatives would like to introduce an exciting program, the ABCs of Diabetes. This FREE program is open to all diagnosed diabetic patients covered under the Polk County School Board Insurance (Blue Cross/Blue Shield of Florida). This intensive program will promote diabetes awareness through educational classes, preventive screenings, and one-on-one Risk Intervention Sessions. The ABCs of Diabetes will provide all of these services at no cost to covered insurance plan members. Plus, participants will be given significant discounts on diabetes prescription medications and prescription supplies.
Participants must attend four 90-minute classes* concerning the following topics:
Participants must attend two Diabetic Screening Saturdays sponsored by Polk County School Board Wellness Program during the year. At these events, participants will receive their HbA1C (hemoglobin) test, eye exam, lipid profile, kidney function test, blood pressure test, and foot exam. Participants must also attend one-on-one Risk Intervention Sessions after the receipt of the participants’ lab results from the Diabetic Screening Saturday.
Participants must complete the ABCs of Diabetes “ABCs to Success” Checklist, which follows the American Diabetes Association best medical practices for diabetics. As outlined on the checklist, participants are required to obtain twice yearly HbA1c (hemoglobin) tests, yearly eye exam, twice yearly lipid profiles, twice yearly kidney function tests, blood pressure tests, and yearly foot exams. Participants can receive these FREE tests at the Diabetic Screening Saturdays, or they may show proof that these tests were completed by their Primary Care Physician (subject to copay and annual deductible).
