FLDOE Survey For Students With Disabilities

The Polk Public Schools Department of Learning Support has an important announcement about the Florida Department of Education’s (FLDOE) annual parent survey concerning student individual education plans (IEPs).

The FLDOE is no longer mailing a paper copy of the survey to parents with a pre-addressed return envelope. An online survey will now be the primary method of response. The survey is for all parents/guardians with children in pre-kindergarten through 12th grade with an IEP. Polk Public Schools has approximately 11,500 students with IEPs.

The Polk Public Schools requests parents to click here to complete the survey. The survey is available from February 1 to July 1, 2010. The school district has made incentives available for families completing the survey. Parents without computer or Internet access can complete the surveys at their child’s school on school district computers.

Each school has an assigned staff person to oversee completion of the surveys. Call the school or (863) 647-4258 for further information on the surveys. Communications about the new online survey are also being sent to parents.

The FDOE and the U.S. Department of Education use the survey to gauge the effectiveness of Florida schools in IEP implementation and school support of parent involvement with their child’s education.
