This form is designed to help government agencies, the media and the private citizens in Polk County access public records related to Polk County Public Schools. The District understands that it must be accountable to its citizens and their right to know how their tax dollars are spent and how decisions are made.
Please remember that student records have another set of statutory requirements and this is not the link to make that request. Student record requests for current students are made at the student's school site. If you are a former student of Polk County Public Schools, grades Kg-12, and need a transcript for a job application or for continuing your education a request form must be completed before transcript can be released. Please complete the following to initiate your Polk County Public Schools Public Records Request.
The actual cost of duplication will be collected for copies of district public records in a form other than simple paper copies.
The actual labor cost of the personnel providing the service will be collected as permitted by State law if the request requires more than fifteen minutes of clerical or supervisory assistance by District personnel including redaction services.
A special service charge attributable to extensive use of information technology resources and/or labor cost of the personnel completing the request as permitted by State law.
