Personal Health Coaching

Free, confidential services are provided by BlueCross BlueShield of Florida for Polk County School Board employees and eligible dependents with on-going health issues such as Asthma, Diabetes, Coronary Artery Disease (CAD), Congestive Heart Failure (CHF), or Chronic Obstructive Pulmonary Disease (COPD).

Services include:

Access to a Personal Health Coach, who will call you periodically to review your health status, answer your questions, provide encouragement, and support your efforts to manage your condition.
Educational materials that will be mailed to you, and are also available on the program web site.
Health assessments that determine your overall health status and identify areas where additional support and education may benefit your well-being. These assessments are also available on program web site.
Call toll-free, anytime 1-877-789-2583 24 hours a day, 7days a week.
